We have pre-and post-session versions available for both the EYE-DA as well as the EYE-TA. This resource comes pre-populated with the child's results and is available to download from our online data entry site, once reports have been generated. Simply click on the 'Get Report' drop-down menu to access this great resource. This EYE Child Action Plan ensures that all stakeholders have the information they need to fully support a child moving forward.
Note: Please keep in mind that you cannot save any edits when the document is opened in a browser. Make sure to download the action plan to your computer to save your text before printing.
