We have created a set of Scoring Sheet templates that can be used in a variety of ways. They include examples on the reverse side so you can see how other teachers have used each of the templates to record their observations. We have also included suggestions of items that can be assessed with these sheets at the bottom of each template.
These Observation Charts are a great tool to use when assessing children over the course of multiple observations. Simply fill in each circle’s quadrants to represent a score of 1 to 4 for each activity used to assess an item.
You can fill in the Teacher Checklist once you feel confident with the children’s final scores per item. This form can also be downloaded from the Data Entry Site, pre-populated with your children’s names. Once you’ve entered some scores, you can generate an updated version that includes all scores you’ve entered.

This checklist includes only the items from the short version of the EYE-TA. If you plan to assess any optional items, please use the complete Teacher Checklist containing all the items instead.
