Check out our EYE Child Actions Plans! We have pre-and post-session versions available for both the EYE-DA and the EYE-TA. This resource comes pre-populated with the child's results and is available to download from our online data entry site.
Once reports have been generated, simply click on the 'Get Report' drop-down menu to access this great resource.

EYE Child Reports are available in 21 languages!
Don't forget in the online data entry site, we we have multi-lingual EYE Child Reports to engage families whose first language is not English. We are continually looking for ways to make it easier for you to discuss every child's progress with families.
Note: Please keep in mind that you cannot save any edits when the document is opened in a browser. Make sure to download the action plan to your computer to save your text before printing.
