User Guide – Post EYE-DA Individual Child Action Plan
This resource is meant to be used once the Post EYE-DA child reports are generated. It can be completed by you (the child’s educator) or during a collaborative meeting with your multidisciplinary team.
Modify or adjust this resource to reflect the supports that are available to you. Note that the cells are fillable and expandable when used electronically.

Page 1 – Information and Results - This is where you will record the information you currently have for the child:
- Fill in the date and the child’s name, age, and group/educator information.
- In the table, record the child’s Pre and Post domain results as found on the child’s report.
- Below each result, add any notes pertaining to that domain.
- In the box at the bottom of the page, include information about the child that was not captured in the EYE-DA assessments or any overall observations.
Page 2 – Interpreting your Post-EYE-DA Results and Other Information - This is where you will consider how well your plan worked and make suggestions for the kindergarten teacher who will work with the child next year:
- Review the information from Page 1.
- The first fillable row is to highlight what worked well (improvements in domain scores for example). Record what worked well and offer suggestions for the kindergarten teacher in the cell to right.
- In the next row, consider modifications to the action plan and identify the strategies that did not have the intended results (i.e., scores remained the same or only increased minimally). Indicate those specific action items and strategies that should be reconsidered. Offer alternate ideas for the kindergarten teacher; this could be a simple adjustment or a whole new idea.
- Include any notes that could be useful for the child’s next teacher.
To edit the Child Action Plan, navigate to your class list page and click on the “Get Report” drop-down menu:
There are two ways that you can edit the Child Action Plan:
- Download the report by clicking on English or French, saving the PDF locally to your computer, and editing it using a PDF reader. OR
- (Recommended) Select “Click here to edit”, which will then expand into a user-friendly window where you can edit, save, and download the Child Action Plan. You can come back and make additional edits at any time. Being able to edit and save directly within the application allows you to keep everything in one place, and easily share and collaborate with your multidisciplinary team!
