How do I generate a child's EYE-DA report? Print or save as PDF

After reviewing your scores for a child, if you have no missing scores, you may proceed in generating the Child Report:

1. Select the Data Entry icon to see the list of children.

2.   A "Generate Report" message is displayed once all the scores had been entered into the system.

Note: Once you have generated a report, you cannot go back and edit any of the scores entered for that child.
