We have pre and post-session versions available for both the EYE-DA as well as the EYE-TA. This resource comes pre-populated with the child's results and is available to download from our online data entry site, once reports have been generated. This EYE Child Action Plan ensures that all stakeholders have the information they need to fully support a child moving forward.
Note: Please keep in mind that you cannot save any edits when the document is opened in a browser. Make sure to download the Action Plan to your computer to save your text before printing.
EYE Child Reports available in 21 languages
Don't forget in the online data entry site, we have multi-lingual EYE Child Reports to engage families whose first language is not English. We are continually looking for ways to make it easier for you to discuss a child's progress with families, so please let us know how this is working for you.
