Filling out the EYE Implementation Form Print or save as PDF

Prior to the start of your EYE implementation, your Client Success Manager will send you an EYE Implementation Form to fill out. This form contains valuable information on how we can better support you during the course of your session.

We have a form to fill out for information on your EYE-DA implementation and on your EYE-TA implementation. Depending on what you have purchased you may have to fill out just one, or both forms. 

This form is very important as it will provide both the CSM and Product Delivery Team with the following insights:

  • Implementation timeline for your entire session (Pre and Post Session Start and End dates).
  • Supply needs (EYE-DA only)
  • Primary and Secondary contacts
  • Type of Excel sheet used to transfer student information, whether a Child (we enter all child information) or No Child Upload (Teachers enter all child information).
  • Other Session Requirements

This form is divided into 4 sections, please note that anything marked with a red asterisk is a mandatory field.

This section will ask you for jurisdiction and primary or secondary contact that will help the CSM and the EYE team to contact for EYE verifications, invoicing, and service information.
 



This section will also ask you if the Primary Contact would like to have “Session Administrator” access. This newly added feature provides the coordinator with greater visibility and control over their session. Session Requirements

session_requirements.pngThis section provides the delivery team with your session specifications. Such as what kind of Child report you request, the preferred colour choice on the reports, the type of excel sheet you would want us to send, the session start and end dates (pre and post if applicable), the type of student ID, and whether the Student ID is mandatory or not.

The Student Identification Number has been a common pitfall for Coordinators when filling out excel files. For this instance, the Student ID is the unique number assigned to each student upon registration in education for the purpose of record keeping. In other provinces, this is called the Learner Registry or the Learning ID.

This number is usually a permanent, unique identifier for each child that does not change from year to year (e.g., Student Number that will stay with the child for his/her entire academic career).

If  Yes is selected then the type of ID you choose should match the student ID number you provide under the Unique ID column when filling out your EYE excel sheet.

Some districts will supplement, or replace,  this ID with their own unique child ID taken from another source (ie PowerSchool, Aspen). If this is an alternate ID you would like to choose, please select Other in the dropdown.

Within this section, we will also require your intended pre-session start and end dates, and if applicable, our intended post-session start and end dates.

This section will ask you the number of kits or booklets that you require and the shipping information for us to process and send your order.

This asks you your preference on how we can reach out to send some product information, resources, surveys, and other updates for EYE.
