Filling out the EYE excel file can be confusing, so we have prepared this article containing all the information you will need to know in order to fill out the excel spreadsheet for your EYE sessions.
The EYE excel file is divided into two spreadsheets. Let’s discuss the first sheet, which is the Student Data. This sheet contains the following sections:
Please note that this Unique Identifier must coincide with the identification selected when filling out the Implementation Form.
Alternate ID (Optional): If you wish to add additional ID numbers for children (e.g., school issued ID), please enter them here.
Child’s Name - Please do not use any special characters when entering the children’s names.
- Last Name (Required): Child’s surname
- First Name (Required): Child's given name.
- Middle Name (Optional): Child's middle name(s).
Sex (Required): Child's gender (e.g., M, F or Male, Female). You may leave the column as blank for children’s whose gender is “In another way”
Date of Birth (Required): Please ensure that the day and month are placed in the appropriate column, as the EYE assessment is based on the child’s age norm and interchanging the date of birth will have a large impact on the child’s scores.
Postal / Post / Zip Code (Optional): If available, please provide the postal/zip/post code for the child's home address.
- Last Name (Required): Teacher's Surname. If a class is taught by more than one teacher please list both names, separated with a slash "/" (e.g., Smith/Jones).
- First Name (Required): Teacher's given name. If a class is taught by more than one teacher please list both names, separated with a slash "/" (e.g., Sarah/Tony).
- Email (Required): This is simply the Teacher's district email address. If a class is taught by more than one teacher please separate both email addresses with a slash "/" (e.g., sarah.smith@eye.ca/tony.jones@eye.ca).
Please ensure that the spelling within the provided email address is correct, otherwise users will not receive their EYE login information once the session is opened.
Classroom Name (Required): The name of the classroom (e.g., K3, AM/PM). This name will appear on the individual child reports. Please ensure that the classroom names are unique within each school in the jurisdiction. For teachers with more than one class, please ensure that classroom names reflect this (e.g., J Wood AM, J Wood K2).
Language Used For Evaluation (Required): The language used to administer the assessment to each child in the class (English, French or Spanish). Please enter only one language for each classroom.
Please note that this column is available for the EYE-TA excel file.
- School Name (Required): We highly recommend adding the school’s full name and not the abbreviation. This ensures all naming conventions are consistent on a year over year basis, as well as making it easier to support teachers/evaluators.
- School ID (Optional): If available, please you can provide the school's ID number here.
- Postal / Post / Zip Code (Required): This is the school's postal/zip/post code.
- Jurisdiction Name (Required): The name of the district/division/council/network/etc. that the school(s) belong to.
- Jurisdiction ID (Optional): If available, please provide the ID number of the district/division/council/network/ etc.
Please note that the school name entered here needs to match the name we already have in the EYE application, if applicable. We highly recommend addressing the school by its full name and not an abbreviation.
Additional Information: This section is for any additional data you wish to collect.
Note: Additional data is not a required field. Any data entered here will only show up on raw data or specialized reports. Please contact us at eye-support@thelearningbar.com if you have questions about these field
The second sheet, which is the Additional Personnel, is where you will enter the Information for extra EYE users.
Please note that we require this to be filled out for every new session.
Level of Access Information
- School Names (Required): If the person to be added requires access to all schools, simply type “All” in this column. Otherwise you can indicate the specific school that they will need access to. Users can also have access to multiple schools, however they must be separated with a slash “/” (e.g., Montgomery School/Bright Academy School/Dawn School).
- Jurisdiction Name(s) (TA Only) (Optional): Only enter information here if your organization contains multiple jurisdictions. Please provide a list of the jurisdictions this person will need access to. Separate multiple jurisdictions with a slash "/" (e.g., Jurisdiction A/Jurisdiction B/Jurisdiction C). If this person requires access to all jurisdictions you can simply enter "All".
- Evaluating Children (DA only) (Required): Indicate Yes or No if the user requires to assess and enter data for the children
- Viewing Reports: Indicate Yes or No if the user requires to assess to view reports.
Please note that you do not need to include the names of the classroom teacher(s) as their access has already been indicated in the Student Data File. Only add additional teachers here if they reuqire access to the school's report.
